Questionnaires

Several questionnaires were administered to participants near the time of enrollment. Basic data were collected on eligibility criteria such as cigarette smoking, age, and several other criteria using the following forms:

After being determined eligible, participants completed more extensive questionnaires about demographics, medical history, and health habits using the following forms:

• LSS MHQ (PDF - 193 KB)
• ACRIN DP (ACRIN NLST CRF Set)
• ACRIN SS (ACRIN NLST CRF Set)
