Effect of risk-stratified screening with flexible sigmoidoscopy on colorectal cancer incidence and mortality
Summary
Globally, colorectal cancer (CRC) ranks as the third common cancer and the second leading cause of cancer mortality. Flexible sigmoidoscopy (FSG) is currently the only screening modality that have been proven to be effective in reducing CRC mortality based on randomized controlled trials (RCTs). Although most current guidelines recommend population-based screening, it is unaffordable and unsustainable for several countries due to large coverage of population, inaccessibility of FSG, lack of insurance coverage, lack of professional screening technicians, and low compliance. CRC screening in high-risk groups has been suggested as a more cost-effective and sustainable choice compared with population-based screening, especially in resource-limited countries. Although various CRC risk prediction models have been developed to identify high-risk groups of CRC, most available CRC screening guidelines still recommend screening based on age, family history, and genetic susceptibility. Moreover, several studies have investigated the effects of risk-stratified CRC screening strategy in observational studies, few studies investigate the effects in intervention study.
Therefore, we aim to develop and investigate the effects of risk-stratified FSG screening on CRC incidence and mortality based on the long-time follow-up of the Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial.

Aims
1) To develop a CRC risk prediction model to identify high-risk group of CRC.
2) To compare the subgroup effects of FSG screening on CRC incidence and mortality between the screening arm and the usual-care arm.
3) To explore and compare the FSG screening by intensity among different risk groups in the screening arm.
